Before and During Your Appointment

Before your appointment, come a little early to our office to fill out the necessary paperwork, including our New Patient form, Privacy Practices Acceptance form, and HIPPA Information form. Follow the buttons to print your forms if you are able to complete them prior to your appointment.

When your appointment begins at Linzmeier Chiropractic, a thorough health history and physical examination will be given by Dr. Jessica Gonzalez-Monroe. During this time, you may explain all of your concerns and questions about your personal health care to Dr. Jessica Gonzalez-Monroe. You may also receive an in-office X-ray to provide additional evaluation for your condition. Recent X-rays from other physicians are often accepted as well.

After Your Initial Consultation

After reviewing your paperwork, health history, and examination, Dr. Jessica Gonzalez-Monroe will begin to discuss the details of your condition with you and subsequent treatment options to help remedy your pain or other symptoms.

After your first treatment, you may schedule additional appointments, according to your recommended treatment plan. If Dr. Jessica Gonzalez-Monroe does not feel your ailments can be helped by chiropractic care, she will refer you to another type of treatment.
